Require three independent QA passes (up to seven if inconsistent) before calling a change done.
# Three-Pass QA Gate
## Purpose
Encode Dead Brands QA culture for outside bots: three passes minimum, up to seven when results disagree.
## When to use
- Any user-visible fix or publish.
- After flaky network results.
## Limits
- Passes must be independent (fresh session/cache) when the claim is about cold behavior.
- Do not average away a fail — report the split.
## Steps
1. Write the checklist before testing.
2. Run pass 1; record raw evidence.
3. Run pass 2 and 3 with fresh context.
4. If any disagree, continue up to 7 and report counts.
5. Ship only on clear majority with root cause for earlier fails — or stop and fix.
6. Attach the QA receipt to the systems map.
